Pat Butler Cross-Country: Required Forms and Friday Meet Information

Pat Butler Cross-Country Families,

Please review these important reminders for our first meet tomorrow, Friday, September 4, at Kermit King. The meet begins at 2:45 PM.

Required forms

**Medical Authorization Form:** Athletes must have a completed paper Medical Authorization Form on file before they can participate. If your athlete's form is still incomplete, please complete it and return it to the Pat Butler school office as soon as possible.

- **Jersey Agreement:** Please complete, sign, and return the Jersey Agreement before tomorrow's meet.

If you have already returned either form, thank you. No further action is needed for that item.

Transportation and pickup

- A bus will take athletes from Pat Butler to Kermit King.

- A parent or guardian must pick up their athlete at Kermit King no later than 4:00 PM.

- There will not be transportation back to Pat Butler after the meet.
